Overview: The Social Worker provides psychosocial support and assistance to patients and families within a healthcare setting. This role involves assessing patients' social and emotional needs, coordinating care, providing counseling, and linking patients with community resources to support their overall well-being and facilitate their treatment.

Key Responsibilities:

  • Patient Assessment:
    • Conduct comprehensive assessments of patients' social, emotional, and financial needs related to their medical conditions.
    • Identify and evaluate factors affecting patients’ health, including family dynamics, mental health, and socio-economic challenges.
  • Care Coordination:
    • Collaborate with healthcare providers, including physicians, nurses, and case managers, to develop and implement effective care plans.
    • Coordinate referrals to community resources, support services, and financial assistance programs.
  • Counseling and Support:
    • Provide emotional support and counseling to patients and their families to help them cope with the impact of illness, injury, or hospitalization.
    • Facilitate discussions about treatment options, advance directives, and end-of-life decisions.
  • Resource Navigation:
    • Assist patients in navigating the healthcare system, including understanding their rights, insurance benefits, and available support services.
    • Connect patients with community resources, including housing, transportation, and financial assistance.
  • Crisis Intervention:
    • Address and manage crisis situations, including providing immediate support and intervention for patients experiencing acute distress.
    • Develop safety plans and crisis management strategies as needed.
  • Documentation and Reporting:
    • Maintain accurate and timely documentation of patient assessments, interventions, and outcomes.
    • Prepare reports and summaries for case reviews and multidisciplinary team meetings.
  • Education and Advocacy:
    • Educate patients and families about available resources, coping strategies, and support services.
    • Advocate for patients’ needs and rights within the healthcare system and community.

Qualifications

Educational Requirements:

  • Bachelor’s Degree:
    • Bachelor of Social Work (BSW) from an accredited program.
  • Licensure (Preferred but not required):
    • State licensure or certification as a Social Worker (e.g., Licensed Bachelor Social Worker (LBSW)) is preferred but not required.
  • Basic Life Support (AHA or ARC with hands on training).

Professional Experience:

  • Experience in Social Work:
    • Typically 1-3 years of experience in a healthcare setting or medical social work is preferred.
  • Experience with Diverse Populations:
    • Experience working with diverse patient populations and understanding their unique needs and challenges.

Skills and Abilities:

  • Clinical Social Work Skills:
    • Knowledge of psychosocial assessment, crisis intervention, and therapeutic techniques.
  • Communication Skills:
    • Excellent verbal and written communication skills for interacting with patients, families, and healthcare professionals.
  • Organizational Skills:
    • Strong organizational skills with the ability to manage multiple cases and prioritize tasks effectively.
  • Problem-Solving:
    • Ability to identify and address complex social and emotional issues impacting patients and families.
  • Empathy and Compassion:
    • Demonstrated empathy, compassion, and understanding in providing patient and family support.
