Job Description:

As a Flourish Crisis Response Street Outreach Specialist, you will provide street outreach case management and service coordination to youth and young adults enrolled in Flourish's street outreach program. This entails building relationships with youth and young adults experiencing unsheltered homelessness to support their well-being goals and access stable housing. You will conduct needs assessments, provide immediate basic needs support, coordinate care through service referrals, and collaborate with partner agencies.



Job Duties



  • Ensure the delivery of basic needs items to unsheltered youth and young adults in the 16-county service area, either directly or through coordination with partner agencies. Identify and address basic needs effectively when crises may arise.


  • Engage and build trust with unsheltered youth and young adults to establish strong rapport. Build and maintain positive relationships with youth and young adults, families, schools, nonprofits, and business partners to strengthen the network of resources and positive relationships.


  • Conduct comprehensive needs assessments, identify risk factors, barriers, and strengths through various assessment tools and regular check-ins. Collaboratively set and adjust goals, while providing ongoing support and guidance to develop and enhance the skills necessary for successful goal achievement.


  • Implement the Coordinated Entry process for youth and young adults, adhering to Homeless Management Information System (HMIS) standards and completing required trainings.


  • Advocate for and assist youth and young adults in accessing community resources, including SNAP and Medicaid, by providing application support and navigating complex systems.


  • Assist youth and young adults in applying for employment, housing, and other programs that address their needs.


  • Document case management activities, track client progress, and generate reports. Continuously monitor and evaluate client progress, adapting strategies to ensure the most effective and efficient support.


  • Consistently adhere to Flourish's trauma-informed clinical case management model, best practices, and all administrative requirements with support from the Housing Manager, Support Specialists, and supervision provided by the Executive Director.


  • Actively contribute to making the team and culture stronger by assisting with other duties as needed, excluding licensed activity.

  • Bachelor's Degree or higher in relevant area of study (Social Work, Education, Sociology, Human Development, Public Health, etc.) and/or experience working with youth in public education or other community human service organizations.


  • Experience working with marginalized youth and/or families.


  • Skilled in public speaking and effective presentation.


  • Project management and/or program coordination experience.


  • Minimum of one year of social work or relevant experience is strongly preferred.


  • Proficient in basic computer technologies, appropriate for an internet-based company.


  • Regular and predictable attendance.


  • Valid driver's license and responsible driving history.


  • Must be willing and able to obtain a Class E chauffeur's license within 30 days of employment.
