Patient Populations: Hematology Patients (bleeding disorders, HHT, anemia) not cancer more so chronic conditions




Hereditary Hemorrhagic Telangiectasia: pediatric, adult, and older adult patients



Bleeding Disorders: pediatric and young adult patients including hemophilia patients.



Health Education



  • Assess needs and assets for health education resources, projects, and programs within the hemostasis and thrombosis center.

  • Plan, implement, and evaluate health/disease-specific education projects. Manage short and long-term projects and execute their deliverables.

Develop CCHMC educational materials and health topics for the external website related to bleeding disorders. Work with the CCHMC Clinical Content Committee to ensure that materials are written at an appropriate health literacy level. Review and update educational material as needed.


  • Develop and coordinate educational programs for patients and families. Conduct program evaluation as needed.

  • Update the hemophilia educational website, Health for Hemophilia, and the electronic educational resources on the My Conditions page within MyChart.

  • Collaborate with the nurse care managers on supporting the educational needs of the patients and families on projects as needs arise.




PR, Communications



  • Participate in patient newsletter(s) planning committee. Coordinate and collaborate with newsletter writer and designer. Strong written skills, providing health ed to patients, materials for patients and families.



  • Act as liaison with marketing related to patient stories, development of health topics, blog stories, patient photography, and videos.

  • Moderate CCHMC Bleeding Disorders Parent Facebook Group




Hemophilia Comprehensive Clinic:



  • Meet with patients and families in weekly clinic to provide supplementary education on their bleeding disorder

  • Assess patient and caregiver learning needs/preferences. Communicate learning needs/preferences to the medical team.

  • Provide patients and caregivers with updated educational materials and teach key points.

  • Develop and update personalized PowerPoint presentations to be forwarded to the patient's school.


Qualifications:



Bachelor's degree and Ohio license in a clinical discipline (Registered Nurse preferred). Certified Health Education Specialist (CHES)




  • 0+ years of work experience in a related job discipline


  • Ohio license in clinical discipline preferably nursing
