Education: Bachelors or Associates



Job Description:

• Provides direct care and case management of assigned mental health patients in their home
• Consistently assesses and evaluates patient's status and goals and revises the plan of care as necessary
• Maintains and updates schedules of all disciplines in patient's home

Qualifications/Experience Needed:

• Current RN license in Massachusetts
• Open to recent graduates and experienced nurses
• Must be a licensed driver with automobile
• CPR Certified
