The Nurse Practitioner (NP) will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey. This position offers the opportunity to make a significant impact on patient outcomes and promote continuity of care. Visits will be completed in-home and/or community base services such as assisted living facilities, skilled nursing facilities, group homes, or other delegated locations.
Essential Functions
In-Home Provider Visits: Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management with a minimum of 25 visits per week.
Patient Assessment: Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
Diagnosis and Treatment: Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
Chronic Disease Management: Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
Medication Management: Prescribe medications and adjust dosages as necessary while educating patients on proper medication administration and potential side effects.
Preventive Care: Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
Patient Education: Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
Collaborative Care: Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
Primary Care Provider of Record: Serve as patients' primary care provider.
Documentation: Maintain accurate and up-to-date electronic medical records (EMR) to track patient progress, treatment plans, and interventions to be completed within 48 hours of visits.
Bedside Manner: Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
Follow-Up: Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
Quality Assurance: Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
After-Hours On-Call: Standard business hours are Monday through Friday, 8 am - 5 pm, however, this role may be required to participate in after-hours, on-call to conduct longitudinal care and IHA visits as needed.
Travel: Ensure reliable transportation to and from member appointments, adhering to scheduled times.
Compliance: Adhere strictly to all company policies, procedures, mission statement, values, Code of Ethical Business Conduct, and Compliance Program.
Legal Compliance: Comply with all Federal and applicable State and local laws and client Policies and Procedures regarding privacy, confidentiality, and security of health information, and other designated information.
Other Duties: Perform other duties as assigned based on business needs.
Qualifications
Licensure: Active, unrestricted current licensure as a Nurse Practitioner in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
Certification: Certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner.
Provider Status: Obtain autonomous provider status as required by the state you are licensed within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
DEA License: Active Drug Enforcement Administration (DEA) license.
BLS Certification: Current Basic Life Support Certification for healthcare providers.
Clinical Skills: Strong clinical assessment and diagnostic skills.
Communication Skills: Excellent communication and patient education abilities.
EMR Proficiency: Proficiency in electronic medical record (EMR) systems.
Insurance: Valid proof of sufficient car insurance coverage for the state that appointments are scheduled to be completed in with access to reliable transportation.
Driver's License: Valid and active driver's license for a minimum of 3 years without restrictions, and a minimum of 7 years with no DUIs or other felony driving convictions.
Background Check: Ability to pass a background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Physical Demands: Ability to lift and carry up to 20 lb.
Mobility: Ability to sit, stand, bend, and/or kneel to perform assessments. Constant ability to remain in a stationary position, move about inside and outside of member homes, navigate stairs and a variety of dwelling conditions, communicate and exchange information with others, inspect information, perform repetitive motion with arms and fingers, and drive to and from member homes.
Mental Demands: Constant ability to interpret data, problem solve, make decisions, and organize and plan.
Work Environment: Work is performed in member homes and potentially exposed to adverse environmental conditions.
Competencies and Skill Requirements
Self-Motivated: Must be a self-motivated professional who enjoys working independently and making a meaningful impact on member health.
Computer Skills: Must have strong computer skills, and both knowledge and proficiency with electronic medical records.
Risk Adjustment Models: Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
Documentation: Ability to document assessment findings accurately and thoroughly.
