We are currently seeking an Travel RN Case Manager for our client in Medford, OR!


As an RN Case Manager, you will be responsible for assessing patient needs, developing personalized care plans, and coordinating with healthcare providers to ensure comprehensive and continuous care. Your role involves advocating for patients, utilizing community resources effectively, and monitoring patient progress to adjust care plans as needed. Additionally, you will educate patients and their families on healthcare options and provide emotional support throughout their care journey.
