Pay: $17.64-$21.09




Schedule: Monday-Friday 8:00 AM-4:30 PM



The Social Worker Assistant is to support the Social Service Department in the care and treatment of participants entrusted to the facility and to attain or maintain the highest practicable physical, mental and psycho-social well-being of each resident. To serve as a liaison and role model to the community for the facility. Represents the Social Service department, other care center staff, interdisciplinary team members and the facility as a whole to the community. Upholds the values and mission of Volunteers of America National Services to peers and the public. Customers include PACE participants and their families, local healthcare professionals who work with the facility and staff, and the facility staff managers.

  • Licensed where applicable by the State; State requirements may dictate supervision and degree type.

  • High school diploma or equivalent education required;

  • Knowledge of long-term health care and public health settings and familiarity with Federal and State regulations and requirements for long-term care.

  • Excellent interpersonal communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.

  • Ability and willingness to work in various locations, for planned activities or special events, family meetings and conferences, admission calls, admissions and tours




Preferred Experience:



  • Bachelor's degree in Social Work or related degree in social sciences

  • Minimum of one year experience in a long-term healthcare setting

  • Bilingual


Essentials:



  • Supervise and direct local Center Director/Director of Operations

  • Assure Compliance with all PACE Regulations with collaboration and support from VP of Compliance and Excellence and Regional Quality and Compliance Team; assure internal compliance audits are completed and results acted upon to assure complete compliance

  • Assure financial solvency and achievement of financial performance with collaboration and support from Director, Health Plan Operations

  • Assure Achievement of enrollment goals with support from Community Outreach and VP, Strategic Partnerships

  • Development of an annual Quality Plan approved by the local Board of Director; reporting metrics as required to internal resources, external regulators and governing bodies; execution of action plans to meet quality standards and sustain improvements

  • Effective interdisciplinary team functioning to maintain PACE Model fidelity and successful participant outcomes

  • Effective primary care systems and functions in collaboration and support from the Regional Clinical Care Consultant and other Regional Executives; this includes utilization of emergency, hospital, outpatient and pharmacy services and accurate diagnostic coding for Medicare reimbursement
