Your Role:
As a Physical Therapist at Lakeport Post Acute, you will:
Evaluate residents and develop personalized treatment plans to enhance mobility, strength, and function.
Provide hands-on therapy sessions to improve quality of life and achieve care goals.
Collaborate with interdisciplinary teams to create comprehensive care plans.
Educate residents and families on therapy techniques and progress.
Document care and ensure compliance with all regulatory standards.
What We're Looking For:
Current California Physical Therapist License in good standing.
