The Judge Group is looking for Nurse Practitioners to conduct in-home health assessments in Grafton County, NH with one of our largest clients nationwide.
Per diem position offers you the chance to work independently, with compensation for each assessment, and flexibility with scheduling.
Pay is dependent upon experience as a provider.
Mileage is reimbursed at the federal rate
Overview:
Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.
Provide education on medication management, disease processes, and preventive care.
Identify and document diagnoses to support care management and active treatment.
Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.
Minimum Requirements:
Completion of at least 20 hours of in-home assessments per month.
Valid Nurse Practitioner license in the state of New Hampshire.
Board Certification from AANP or ANCC in Family, Adult-Gerontology, or a related specialty.
Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
Strong communication skills and a commitment to delivering compassionate care.
Reliable transportation for travel within the assigned areas.
