Assessment and development of an individualized plan of care through initial and periodic assessments and through communication with patient/family, physicians and other staff. Identifies physical, psychosocial, spiritual and social needs and coordinates the development and implementation of a plan of care to meet those needs effectively and efficiently.
Maintain close communication with the Medical Director Joint Replacement Center (JRC) to evaluate all barriers to care, including medical, social and economic.
The Nurse Navigator will be an integral part of the planning process from the point that a patient is identified as needing a total joint throughout the recovery period.
Conduct mandatory pre-op classes for all total joint replacement patients, inpatient and outpatient, as needed to meet the needs of the patients.
Collaborate with the manager of the JRC to assure compliance with goals of JRC and consistency in application of established pathways.

Qualifications
Registered Nurse licensed to practice in the State of Maryland. BSN preferred or completion within 5 years.
Five years clinical experience in a health care setting and experience in teaching in the health care environment.
National Association of Orthopaedic Nurses (NAON certification or attainment within 2 years.
