Key Responsibilities:

• Assist in coordinating appointments and transportation for residents.


• Facilitate hospice referrals and support end-of-life care processes.


• Collaborate in planning and conducting care conferences with residents, families, and staff.


• Maintain accurate and timely documentation related to resident services.


• Provide emotional support to residents and their families, promoting well-being and satisfaction.

• Compassionate and empathetic individuals dedicated to resident-centered care.


• Strong organizational and communication skills.


• Experience in social services or long-term care settings is a plus.


• Ability to multitask and manage time effectively.
