Lincoln Meadows Care Center is seeking compassionate Physical Therapists (PT) or Occupational Therapists (OT) to join our dedicated team in providing high-quality care to our residents. We have PRN, part-time, and full-time positions available with weekend availability required. Our facility is committed to enhancing the quality of life for our residents, and we are looking for team players who share that passion.

Key Responsibilities:

• Provide physical or occupational therapy services to elderly residents


• Develop and implement personalized therapy plans to enhance resident mobility, independence, and quality of life


• Collaborate with a multidisciplinary team to ensure comprehensive care


• Maintain accurate documentation of therapy sessions and progress


• Support residents and their families through education and compassionate care

Qualifications:

• Valid Physical Therapist (PT) or Occupational Therapist (OT) license in the state


• Experience in a skilled nursing facility or with elderly patients is a plus


• Strong interpersonal and communication skills


• Ability to work independently and as part of a collaborative team
