The Substance Use Counselor for the Assertive Community Treatment (ACT)Team provides intensive intervention, Treatment, rehabilitation, and support services to consumers with severe, acute and persistent addictive diseases. The intensive treatment and case management services, provided as part of a comprehensive individual service plan, are designed to serve the acutely destressed consumer in the community with no delay, preventing hospitalization, incarceration or other placements.
Under broad supervision, counsel with emphasis on prevention and/or behavior modification. Work with individuals and groups to promote optimum mental health. May help individuals deal with addictions and substance abuse, family, parenting and marital problems, suicide, stress management, problems with self-esteem and issues associated with aging and mental and emotional health. May serve as team leader.
Duties & Responsibilities:
Provides intensive intervention, treatment, rehabilitation, and support services to consumers with severe, acute, and persistent addictive diseases. Provides a comprehensive individual service plan, designed to serve the acutely destressed consumer in the community with no delay, preventing hospitalization, incarceration, or other placements.Conducts clinical assessments and provides direct clinical treatment, rehabilitation, professional case management and support services as needed. Assumes primary responsibility for developing, writing, evaluating, and revising overall treatment goals and objectives in collaboration with the consumer and ACT team members. Continuously evaluates consumer care and progress to ensure immediate attention to changing addiction treatment needs and revision of treatment plan as warranted. Functions as liaison to community agencies and families to maintain coordination of treatment processes. Maintains medical records and complies with third-party payer requirements for documentation and authorizations.Provides accurate and timely documentation of charts in a computerized record system. Bills for services hours according to agency standards.
Program Specific Duties:
Travel in all Region 3 and transport individuals when needed.Perform nursing duties.Work after hours and on weekends when needed.Be available to respond to emergencies 24 hrs. a day when On-call.Participate in daily morning meetings.Perform other discipline duties when appropriate and needed.Other duties as assigned.
Minimum Qualifications:
Master's degree from an accredited college or university AND Two years of related experience OR bachelors degree from an accredited college or university in related field AND Four years of related experience OR One year of experience at the lower level Behavioral Hlth Counselor 2 (HCP101) or position equivalent. Note: Some positions may require certification or licensure.
Preferred Qualifications:
Group/individual experienceOne (1) year Substance Abuse experienceAssessment experienceCertified Addiction Counselor (CAC) IIMaster Addiction Counselor (MAC) or Master and Certified Addiction Counselor 2 (CAC2)Experience working as part of an ACT team.Experience working in community and/or Residential Services.Good oral and written communication skills. Experience entering& managing client information with an electronic medical record system, preferably CareLogic.Strong Commitment to the right& ability of each person with severe& persistent mental illnesses to live in normal community residences, work in market jobs,& have access to helpful, adequate, competent, and continuous support and services.
Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.Other duties as assigned.
