The Clinical Team Assistant LPN position reports directly to Clinical Team Manager and supports both the Clinical Manager and the interdisciplinary team members. Key responsibilities of the role include scheduling, coordinating and organizing all aspects of the clinical team including processing clinical documentation. Serving as a customer resource representative for the interdisciplinary team, caregivers and patients. The Team Assistant is directly accountable to the Clinical Manager, who is directly accountable to the Clinical Director. The clinical team assistant will perform routine home health visits.
Impact of this role in the organization is to assist the interdisciplinary team to meet the clinical and regulatory demands of their role, enabling them to provide enhanced clinical care, improving the overall outcome of the patient, while increasing revenue opportunities.
Responsibilities
Core Responsibilites and Essential Functions
Scheduling a.Reviews & coordinates visit schedules to ensure coverage of each day's visits. b.Identifies and reconciles scheduling problems, refers to manager as appropriate. c.Tracks all pending and hospitalized patients. d.Interacts with Intake staff & healthcare facilities, as needed. e.Schedules all home care disciplines to ensure timely provision of care occurs. Work with physicians to obtain updated orders, to ensure timelines of care. f.Schedules staff according to caseload needs, utilizing PRN staff when needed. g.Maintains various reports. h.Collaborates with other teams to assure coverage for daily, evening and weekend visits. i.Coordinates assignment of patients to clinicians based on specialty, competency and geographic location.
Documentation a.Address variances in identified open/missed visit reports, actively works to minimize issues. b.Track documentation for timeliness of submission. c.Responsible for identifying untimely or incomplete work. d.Notifies clinical manager of deficiencies. e.Verify visits, comparing clinician schedules to patient documentation, to ensure accurate payroll and billing. f.Registers home care patients receiving lab services. g.Assists clinical staff with word processing support for plans of care and recertifications.
Coordination/Patient Care a.Answers phone calls and receive other information (faxes, etc) sent to the team. Determine whether to address situation independently or to forward on to another party. b.Functions as an initial point of contact for interactions with the interdisciplinary team members, managers and patients. c.Daily monitoring of the patient results of their vital signs via telehealth monitoring.
Patient Population a.Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on team. b.Demonstrate knowledge of the principles of growth and development of the life span and processes the ability to assess data reflective to the patient's status and interprets the appropriate information needed to identify each patient's requirements, relative to his or her age, specific needs and to provide the care needed as described in department policies and procedures.
Patient Care a. Perform daily home health visits.

Required Minimum Education
High school diploma Required and
Other Education Graduate from an accredited LPN program Required
Required Minimum Experience
Minimum 3 years work experience working with customers/clients and strong clerical background. Strong computer skills and Microsoft office products Required
Minimum 1 year Home Health experience Preferred
Minimum 1 year Medical/Surgical Nursing Required

Ability to interact appropriately with patients from various backgrounds.
Must be able to build and sustain trust with the physician, Clinical Manager and the interdisciplinary care team.
