1. PATIENT ASSESSMENT: Perform selected, direct, age-specific patient assessment duties and responsibilities under the delegation of a registered nurse with a providers order as indicated for protocols & diagnostic tests.
a. Welcomes patients and introduces self and role, in a manner that is consistent with Express Care service standards and the Language of Caring program. Verify patient identification using two patient identifiers
b. Room patient and assist with triage process by starting the patient interview and assessment, completes accurate patient information to include, but not limited to, vital signs, weight, pain assessment, chief complaint, medical history and reconciled medication list, specific to population served and per policy, in collaboration with the RN
c. Recognizes and reports abnormal physiological monitoring data (vital signs, ect.) and life threatening emergencies, notifying RN/provider promptly
d. Collaborates & communicates relevant patient data to RN/providers clearly and effectively, both orally and in writing
e. Anticipates needs of patient encounter to optimize RN & provider time, assists providers & RN with patient disposition (DME/dressing/splint application & Logicare prep ect.), procedures (equipment & supplies in room and ready for use), & ensure other elements of encounter (paperwork & past encounter information etc.) are made available. Patients in Jonny if applicable.
f. Recognize and report signs/symptoms of patient abuse/neglect or intent to inflict self-inflicted harm, as well as initiates intervention for aggressive/violent patients
g. Recognizes potential need to perform tests (throat swabs, EKG, imaging ect.) per protocol and notifies RN promptly. Competency in performing EKG's, throat and nasal swabs as delegated by RN.
h. Prioritizes and organizes delegated tasks and complete in an efficient and timely manner, requesting and utilizing available resources & tools to meet the patient/family and units needs
i. Competency in performing and documenting visual acuity and GCS
j. Follow policy/procedures to include, but not limited to assessment / re-assessment / protocols / patient flow / pain. Follows clinical practice standards pertinent to care collaboratively with the RN
k. Recognize patients at risk for falls remove any environmental risk (rolling stools, etc.) and notify the RN immediately. Use W/C for at risk patients (leg injury, in need of imaging, etc.) moving about the unit, to minimize risk. Ensure call bell is in reach.
2. PATIENT CARE: Perform selected, direct, age-specific patient care duties and responsibilities under the delegation of a registered nurse with a providers order as indicated for protocols & diagnostic tests.
. Records pertinent, easily legible patient data in the medical record
a. Utilizes Pain Scale and documents per policy, to include, but not limited to, initial assessment of pain, post medication administration & at discharge
b. Perform centrifuge of lab samples, stores samples in the appropriate manner until transported to main lab, disposes of un-used samples per safety standards standard operating procedures (SOP)
c. Competency in performing phlebotomy, labeling of sample per policy and in accordance with regulatory compliance standards. Data enters order into EHR and appropriately documents the collection in the medical record
d. Competency in performing CLIA/point of care testing (w/ annual competency) and maintain QA/QC compliance logs
e. Proactively monitors the patient to ensure that they are seen in a timely and efficient manner. Keep patients alerted to delays and offer comfort measures when applicable. Provides accurate explanations & instructions at an appropriate time and place and at a level of understanding to the patient, parent or family
f. Competency in performing DME application, with pre & post assessment documented in the medical record. Completes required DME paperwork
g. Competency in splinting, wound care & dressings to promote healing and prevent complications, under the direction of the RN
h. Evaluates own performance and recognizes growth opportunities. Shows a desire for new knowledge and skills to enhance service to patient and achieve organizational goals. Seeks constructive feedback regarding his or her own practice
i. Competently performs clinical duties as ordered by a provider and under the direction of the RN may include, but not limited to, EKG's, specimens collection, visual acuity, O2 sat & ear irrigation
j. Prepares, administers and evaluates the effectiveness of selective medications
3. UNIT OPERATIONS: Demonstrates competence in the performance of clerical and in the overall function of the unit
. Perform clerical functions as delegated/assigned to include, but not limited to, accurate and timely coping of daily medical records and completed follow-up forms, filing copies, attaching follow up forms to the copy, coordinating DME forms billing and chart audits & faxing/scanning as needed.
a. Maintains work area in a professional, clutter-free and organized manner to include spaces both seen by patients and not
b. Enters provider orders (Lab, Imaging & Cardiology etc.) into the order entry system timely
c. Assures compliance with Kronos and accurately completes all payroll functions within specified time frame
d. Completes daily room check and maintains adequate level of supplies in patient rooms
e. Maintains a printed supply of T-system, order & Home Medication forms and adequate levels of paper and toner for coping and printing
f. Assists and supports a new or transferred employee during their orientation period
g. Provides timely, accurate and complete documentation of near misses by completing an occurrence report
h. Answers the phone in a manner that is consistent with Express Care service standards and the Language of Caring program. Documents patient telephone conversations and accurately relays information to the RN/provider, for appropriate attention. Quickly identify when the phone call should be handed to an RN.
i. Clean and disinfects exam room and equipment (using appropriate PPE) promptly after patient discharged to facilitate turnaround time, in support of the under 60 minute length of stay goal
j. Coordinates sterile processing standard operating procedure to include daily inspection of soiled instruments and cleaning if needed. Reconcile returned instruments with record of sent items. Contact surgical processing if missing items.
k. Monthly review and documentation for all out-dates (not including Pyxis medications) on the unit
l. Manage linen in the department both soiled and clean daily and as needed. Prepare for collection as scheduled.
m. Manage bio (red) trash & sharps containers in the department as needed. Prepare for collection as scheduled
4. COST: Demonstrates competence in the performance of cost duties and responsibilities.
. Conserve, preserves and economizes expendable resources (supplies, overtime, and equipment)
a. Communicates with Manager/ or designee about availability and management of medical/office supplies, system equipment and personnel resources at unit/department level
b. Flexible with work schedule and anticipate site transfers as needed
c. Completes assignments in an efficient and timely manner, requesting and utilizing available resources & tools in meeting the patient/family and units needs
5. QUALITY: Demonstrates competence in the performance of quality duties and responsibilities.
. Participates in ongoing educational activities and acquires knowledge and skills appropriate to your role (Registered Medical Assistant).
a. Shows commitment to learning new knowledge and skills to enhance service to patients / families and achieve organizational goals
b. Press Ganey: actively pursues improving customer service standards; demonstrates and contributes, working effectively and cooperatively with members of the team to identify opportunities for improvement and actively participates in recommending plans of action to address identified issues
c. Articulates Nurse Sensitive Indicators and unit based quality programs/goals and describe role/contribution in accomplishing expectations
d. Participates in QC/QA/PI initiatives of the organization. Describe the Triple Aim.
Qualifications
Experience Preferred/Desired
• 6 months experience as a Medical Assistant
Education Minimum Required
• High School Graduate
• Successful completion of an Accredited Medical Assistant School/Program
Special Skills Minimum Required
• AHA BLS or CPR
Licensure and/or Certifications Required
• AHA BLS or CPR
• Must be Registered (RMA), Certified (CMA) or Certified Clinical (CCMA) within first year of employment.
