Summary:

Schedule/Shift: PT: 24 hours per week, W, Th, F (3:30pm -11pm)


The Admit/Discharge Coordinator preforms all the essential job functions as the Hospital Admitting Specialist along with assuring that staffing is complete for each shift, creating pending admissions as needed, assures patients are placed in the most appropriate accommodation, facilitate proper placement within the unit, and checks the bed planning tools while resolving scheduling discrepancies according to standard operating procedure. The Hospital Admit/Discharge Coordinator also monitors bed planning system of pending admits, discharges, and transfers in a structured format and reports all information on shift exchange or end of day. The Admit/Discharge Coordinator, under the direct supervision of the Hospital Admitting Supervisor, helps to accurately schedule patient discharge orders and procedures while updating demographic and insurance information, completing pre-registration, accurately registering patients for their appointments, and completes insurance verification while remaining positive, cooperative and supportive to patients, families and fellow colleagues. Essential Job Functions:



  • Assures that work is completed timely and accurately during each shift. Works with employees to correct missing or inaccurate information. Assures bedside registration is completed. Corrects admitting errors and monitors for fraudulent patient activities.


  • Assures that the patient is placed in the most appropriate available accommodation in cooperation with patient care units and Patient Care Coordinator/Charge Nurse to facilitate proper placement with the unit and according to standard operating procedure.


  • Creates pending admissions for hospitalization of therapeutic, diagnostic and invasive procedures, as needed. Obtains required information from the patient/family in order to complete the patient registration, fills out appropriate application forms for patients and documents helpful information, for co-workers in the notes sections, i.e. demographic section, insurance section, etc. Works with security and police to obtain information for patients when needed.


  • Checks the bed planning tools and resolves scheduling discrepancies according to standard operating procedure. Monitors bed planning system of pending admits, discharges, and transfers in a structured format. Reports off all information on shift exchange or end of day.


  • Assures all patient information is collected upon discharge, i.e., accident injury information, demographics. Coordinates all Emergency Department discharges and collects payments. Collects and posts payments, maintains and balances cash drawer, and completes daily deposit .


  • Looks up current insurance and co-pay information using third party eligibility screens in computer. Completes insurance verification through use of coverage search, real time eligibility,Thor, Medifax, or other eligibility software according to standard operating procedures or checks all admits to verify that one has been completed and appropriate changes made to account.


  • Band patient's coming from regional clinics and the STEMI program while also helping direct patients being brought in by EMS. Collaborates with EMS team to receive patient's information and relay information regarding transfers. Directs outlying facilities on patient's placement and necessary paperwork. Notifies charge nurse when ambulance has arrived.


  • Performs other duties as assigned or needed to meet the needs of the department/organization.
