Job Details:
Location: Fully Remote
Hours: Monday-Friday 8:00AM-5:00PM PST
Compensation: Competitive and based on experience
Summary: Conduct telephonic outreach to offer adherence interventions and needed follow up on SCAN members identified as "high risk" through the Pharmacy Department. Support Pharmacy Quality measures set forth by CMS by collaborating with internal & external stakeholders by phone, email and fax to ensure members' access to quality care. Proactively identify member needs, medication adherence gaps and provide coaching on how to be more adherent.
This job will have the following responsibilities:
Conducts telephonic outreach to SCAN members with identified medication adherence issues based on measure specifications set by CMS; assesses for barriers and performs necessary follow up with all applicable stakeholders (i.e. Caregiver, Pharmacy, Provider, Medical Group) to ensure member is adherent to medications.
Documents outreach information in the care management system during phone calls with stakeholders and documents all corresponding information or notes per department policy.
Communicates by phone, email & fax with all applicable stakeholders based on identified health interventions and/or quality initiatives.
Utilizes department desktop procedures, workflows, job aids and training material.
Identifies barriers to work processes and brings to the attention of the supervisor/manager.
Maintains the member's right to privacy and protects SCAN operations by keeping information confidential.
Adheres to all quality, compliance and regulatory standards to achieve HCS and SCAN outcomes.
Actively supports the achievement of SCAN's Vision and Goals.
Contributes to team effort by accomplishing related results as needed. Other duties as assigned
Qualifications & Requirements:
Pharmacy Technician license
3 years of medical assessment experience, or related healthcare experience preferred
Demonstrated knowledge of assessment, health and functional problems of older adults and managed care.
Strong interpersonal skills, including excellent written and verbal communication skills.
Strong organizational skills; Ability to multitask.
Ability to appropriately maintain confidentiality
Strong analytical and critical thinking skills, required.
