Visalia, CA

The Community Assistance, Recovery, and Empowerment (CARE) program is aimed at delivering mental health and substance use disorder (SUD) services to severely impaired individuals who suffer without access to essential treatment. The Case Manager I will play a key role in developing and facilitating linkages, consultations, advocacy, and both individual and group rehabilitation services, as well as supportive housing initiatives in collaboration with the CARE Court team.

How will I contribute
As part of a service delivery team, provide services to mental health, substance abuse and dual diagnosis unhoused individuals to address barriers to engagement. Participate in court hearings and collaborate with justice system stakeholders. Maintain clear, accurate, and concise consumer records in the SmartCare EHR system, in a timely manner. Monitor client progress and communicate with the treatment team. Assistance in improving, maintaining, or restoring an individual or group of individuals functional skills, daily living skills, social and leisure skills, grooming and personal hygiene skills, meal preparation skills, and support resources; and/or medication education Assist with program design and implementation. Coordinate comprehensive care plans and facilitate linkages to outside community services. May encounter risk of working with persons with SMI (serious mental illness) that may include, but not limited to persons in mental health crisis, persons that may be intoxicated or impaired by drugs or alcohol, persons who may be DTO (danger to others), DTS (Danger to Self), or GD (Gravely Disabled). If such situations arise, staff will be required to call local emergency personnel to help deescalate the crisis, use crisis intervention techniques, and consult with supervisor as soon as practically possible. Perform other duties as assigned to support the overall functioning and success of the program

What I bring
High School Diploma (or Equivalent) 1 year of related experience, preferably in a healthcare or behavioral health setting. Lived experience or experience with a family member with mental health and/or substance use recovery; including, but not limited to, mental illness, addiction, foster care, high ACEs score, trauma, intimate partner violence, homelessness, and neurodivergence. Class C Driving License Local resources, information about accessing social welfare systems, advocate on behalf of clients. Skills to provide case management/linkage services to SMI population. Ability to relate to others comfortably and be able to establish and maintain effective relationships. Ability to communicate effectively in writing. Great people skills, patient, and persistent. Have a positive attitude toward disadvantaged and disenfranchised individuals. Able to think and work independently to make decisions when faced with daily challenges. Daily interactions with outreach and clinical staff. Frequent interactions with people from other agencies. The case manager works well with others, is able to manage time wisely, and communicates effectively. College in a related field, Bilingual English/Spanish preferred. Certified Nursing Assistant (CNA) or Medical Assistant (MA) experience desired.
