Located in the heart of World Golf Village, Westminster St. Augustine is currently accepting applications for the position of full-time Health Center Admissions and Social Services Director. This individual will assure that the emotional and social needs of all residents are met and maintained on an individual basis in accordance with regulatory guidelines and our established standards and guidelines. This position will also perform clinical assessments/MDS coding for potential and current Health Center residents. Responsible for communicating with CM/planners and the admissions process as well as the coordination of the care plan.

ESSENTIAL JOB FUNCTIONS:

The Health Center Admissions and Social Services Director must comply with all applicable rules, policies, standards and guidelines related to employment within Westminster and its communities, including:

Prepare comprehensive psychosocial assessments and care plans for all residents.During the Admission process tour facility and available accommodations with potential residents and family members. Review and obtain signatures for required documentation.


  1. Resident and family outreach – advocate for the residents and be the main contact for the family members.

  2. Access to community resources and present residents/family member with health care provider availability; includes home health care, hospice, equipment provider, etc.

  3. Manage discharge planning; responsible for appropriate discharges to a less restrictive setting (outside health services).

  4. Internal contact for resolving resident/family member grievances.

  5. As a Grievance Coordinator maintains a Grievance Log (accept/write grievances on behalf of party); conducts investigations and follow ups with resolutions. Conducts abuse prevention In-services for staff members.

Administrative Functions:

1. Coordinate all admissions (Medicare and other direct admissions) with transfers from hospital or home to the facility.

2. Tour the facility with prospective residents/family members. Assist with referrals as needed.

3. Assist in the evaluation of all potential residents to ensure appropriate placement.

4. Ensure proper federal, state, and facility mandated documentation is completed. Ensure all necessary Medicare and/or insurance information is available and confirmed prior to admission.

5. Review admission packets for accuracy and approval. Enter new resident information into Electronic Charting System.

6. Educate residents and families about the extent of Medicare and Medicaid coverage, including length of stay, amount of coverage, and co-insurance days.

7. Inspect rooms for cleanliness prior to the actual admission.

8. Develop and maintain relations with physicians and hospitals on an ongoing basis, including for example: providing them with Medicare updates and changes in facility procedures.

9. Ensure that the budgeted census does not decrease below the established minimum.

10. Periodically assess services provided by the facility; compare these to the needs of the community.

11. Assess how the current services compare and contrast to those offered by other comparable facilities in the area.

12. Serve as a liaison with the community.

Medical Community:

1. Identify services, treatments, and funding options in the Health Center.

2. Develop and maintain relationships with discharge planners, physicians, and managed care organizations. Communicate with discharge planners to facilitate transfers to the Health Center. Coordinate transfers to the facility from home or other post acute facilities. Communicate bed availability to all interested parties.

3. Communicate to entire team the goals, outcomes, payer authorization, and expected length of stay for the resident.

4. Data collection and reporting as required (daily census).

5. Determine resident satisfaction and compliance with services provided.

6. Attend educational seminars to remain informed of changes in Medicare, Medicaid, MCO, and long term care regulations.

7. Coordinate with Social Services on resident level of care transitions, pay status, bed holds, and discharges.

8. Other duties as designated by supervisor.

ESSENTIAL QUALIFICATIONS:

Education & Experience: Bachelor’s Degree in Social Work required; Master's degree in Social WOrk preferred. 3 Years experience in skilled nursing facility or medically related area is required. 

Physical and Environmental Requirements: Balancing, Kneeling, Reaching, Crouching, Pulling, Standing, Manual Dexterity, Pushing, Stooping, Grasping, Lifting Up to 40 lbs.

Sensory and communicative activities with or without prosthetics of this position are: Feeling, Smelling, Speaking, Hearing, Seeing

An individual in this position will be exposed to: Inside/outside environmental conditions, Bloodborne pathogens, Respiratory infections

Must be able to cope with mental and emotional stress of this position.
