Healthcare Access Maryland’s (HCAM) MATCH Program works in conjunction with the Baltimore City Department of Social Services to ensure all children in foster care receive health services and ensure children’s health needs are met. The Social Work Medical Case Manager provides case management for the MATCH program by collaborating with foster parents, parents, and all other members of the interdisciplinary team to coordinate holistic healthcare for children in the MATCH program.

Duties And Responsibilities:


  • Coordination of care- partner with foster parents, child welfare caseworkers, biological parents, managed care organizations, and healthcare providers

  • Ensure the healthcare needs of children are being met in a timely manner.

  • Identify and monitor children at risk for health-related issues and/or exacerbation of illness.

  • Serve as a POC for resources to address children’s mental health issues.

  • Provide resources and education to children and caregivers to maintain wellness.

  • Attend Case Assignment Meetings

  • Assign New Entrant Cases

  • Assist foster care families with navigating the HealthChoice system in establishing a medical home for foster care child(ren)

  • Prioritize care and contacts based on the needs of children.

  • Document case management assessments, activity, treatment plans and resolutions in CJAMS and MATCH database/data entry.

  • Participate in Family Involvement Meetings as required.

  • Participate in staff, community, professional and inter-agency meetings, and conferences.

  • Adherence to HCAM’s Commitment to Ethics in the Workplace and all other organizational policies and/or procedures

  • Other job duties as assigned.

$2500 with satisfactory completion of the 90 day introductory period

Minimum Qualifications:


  • A Master’s degree in social work from an accredited college/university

  • Current licensure as a LGSW or LCSW-C in the State of Maryland (educational clinical supervision currently unavailable)

  • One-year related experience preferred.

  • Clinical experience with children and families in a public health/community setting preferred.

  • Planning and organizational skills

  • Ability to multi-task

  • Effective interpersonal & communication skills

  • Proficiency in Microsoft Office Suite and other basic computer functions

Work Environment:

While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
