Job Summary

HCAM offers diverse connection and coordination services for individuals and families across Maryland through various programs. HCAM’s MATCH Program (Making All the Children Healthy) provides coordination, education, and advocacy to ensure Baltimore City youth who are in the foster system or reuniting with their families receive the physical and behavioral health care they need through age 21.

The Care Coordinator Supervisor will serve as a part of MATCH’s team by providing direct supervision of the Care Coordinator Associates to ensure that standards of practice are met in working with individual clients and their families.

Duties And Responsibilities:


  • Supervision includes training, auditing, and evaluation of staff who perform the following functions:

    • Ensure the health needs of children in out of home placement are being met by completing timely medical case reviews and ensuring health services meet EPSDT standards

    • Complete electronic documentation of the child’s health services and needs in the child welfare database (CJAMS) and in the MATCH database

    • Serve as a resource person for BCDSS permanency staff for navigating the medical, dental, mental health systems and for making clinical recommendations regarding services to meet the child’s health needs. 

  • Participates in staff, community, professional and inter-agency meetings as requested. 

  • Works directly with Baltimore City Department of Social Services programs to coordinate services for clients within required time frames.

  • Organize, coordinate and develop trainings to ensure staff have the necessary skills to perform their duties

  • Serve as a resource to staff for best practice and care

  • Other Duties as Assigned

Minimum Qualifications


  • Bachelor’s degree required; Master’s degree preferred.

  • A minimum of 3 years of management or relevant leadership experience

  • Knowledge of the foster care system and health field preferred

  • Clinical experience providing services to families and children, preferably in a public health/community setting.


  • Excellent planning, time management and decision-making skills

  • Proficiency in Microsoft Suite, Windows, and other basic computer functions

  • Excellent written and verbal communication skills 

Requirements & Experience: 


  • Bachelor’s degree required; Master’s degree preferred.  


  • A minimum of 3 years of management or relevant leadership experience 


  • Knowledge of the foster care system and health field preferred  


  • Relevant clinical experience providing services to families and children, preferably in a public health/community setting.  


  • Excellent planning and organizational skills  


  • Well-developed interpersonal & communication skills  


  • Professional appearance and demeanor  


  • Computer literate specifically MS Word, Excel, Outlook, and Gmail
