We are looking for a Case Manager for our Outpatient Department. The position will provide referrals for clientele, meet with patients, understand patient needs, and link them to resources with the proper documentation. The position will also:

• Data Entry for audits including the pain assessment, nutrition assessment, PHQ-9 and GAD as indicated as well as tobacco screening. 

• Assist patients in making appointments by providing follow-up phone calls when appointments are missed to track compliance. 

• Assist with collecting information for people who want help with ROI’s.


• Assist with treatment plans, signage and updating of the treatment plan.


• Part of the multi-disciplinary team to assist patients  with:
  
  
  
  •  Applying for food stamps.
  
  
  • Applying or reapplying for Medicaid.
  
  
  • Assist in helping people access Sooner Ride
  
  
  • Disability paperwork
  
  
  • FMLA paperwork


• Other duties as assigned

• Bachelor's degree in related field from an accredited university


• Certified Case Manager Certification


• Ability to effectively communicate in English, both verbally and in writing


• Basic computer knowledge
