Job Responsibilities

Orientation and Identification
- Understand the facility's administrative structure as introduced during pre-training orientation.
- Use wristbands or photo card files to correctly identify residents before administering treatments, serving meals, etc.

Reporting and Documentation
- Report all accidents and incidents during the shift in which they occur.
- Use only facility-approved abbreviations when documenting.
- Notify the Nurse Supervisor or Charge Nurse promptly of any changes in a resident's condition.
- Record all information on flow sheets, notes, and charts in a clear, descriptive manner.
- Maintain confidentiality of resident information, especially protected health information (PHI).
- Immediately report any suspected or known privacy violations to the Administrator.
- Report any unauthorized access or attempted access to the facility's information system.

Care Duties
- Perform only those nursing care procedures for which you've received training.
- Prepare resident rooms for new admissions (e.g., make bed, put up name tags, have admission kit ready).
- Greet new residents, escort them to their rooms, and introduce them to roommates, other residents, and staff.
- Inventory and label residents' personal belongings as instructed. Store their clothing appropriately.
- Help residents pack when being transferred or discharged and assist with transportation to new locations or vehicles.
- Participate in nursing shift reports and follow exposure control policies related to blood and body fluids.

Daily Resident Care
- Make beds and provide extra blankets as needed.
- Take and record residents' temperatures, pulses, and respirations (TPRs) as instructed.
- Weigh and measure residents as needed.
- Frequently check residents who are unable to call for help.
- Respond to resident calls promptly and ensure their personal care needs are being met.

Meal Assistance
- Help residents identify food on their trays, especially those with visual impairments.
- Record food and fluid intake.
- Report any changes in eating habits to the Nurse Supervisor or Charge Nurse.
- Keep water pitchers clean, filled, and within residents' reach.
- Provide after-meal care, including tray removal and cleaning the resident's hands, face, or clothing.

Teamwork and Communication
- Follow all assigned tasks and schedules as directed by supervisors.
- Collaborate with other departments to ensure resident needs are met.
- Report any occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals according to facility policy.

Resident Rights
- Keep all resident care and health information confidential.
- Report any unauthorized disclosures of information.
- Report any allegations of resident abuse or theft of property.
- Respect a resident's right to refuse treatment and notify a supervisor of such requests.
Supervisory Responsibilities- This position does not include supervisory duties.
Qualifications

Education
- Minimum: 8th-grade education

Language Skills
- Able to read, analyze, and interpret business periodicals, professional journals, and regulations.
- Able to write clear reports, business correspondence, and procedure manuals.
- Able to present information and respond to questions from managers and colleagues effectively.

Mathematical Skills
- Able to apply basic math concepts such as fractions, percentages, and ratios.

Reasoning Ability
- Able to solve practical problems with limited standardization.
- Able to understand and follow written, oral, diagram, or schedule-based instructions.

Physical Demands
