SUMMARY
The Population Health Coordinator is a key member of the practice team who has an understanding of primary care practices and assists in the coordination of care required for optimal coordinated population health
RESPONSIBILITIES
Documents all patient interactions in a concise manner that is compliant with documentation requirements
Conducts inbound and outbound calls including, but not limited to: patient scheduling, screenings, reminder calls, panel management
Assists to schedule and coordinate patient transportation, follow-up physician appointments
Utilizes various reports to track patient referrals and scheduling no-shows for patients in collaboration with practice staff
Provides clerical and / or administrative support to clinical staff and managers for special studies, projects and reports
Provides excellent customer service by serving as a resource to all internal and external customers
Maintains knowledge of all health plan benefits and payor requirements,
Call patients as designated by PCP or clinical staff
Verify patients have been seen and follow up on no show patients
Assist all patients with barriers
Ensure appropriate clinical information is communicated to clinical staff when there is variance from plan of care
Coordinates other medical office assistants in outreach and report validation needs
Closes Care Gaps
MINIMUM QUALIFICATIONS
Valid NYS drivers' license
More than one year of related experience in a medical setting
PREFERRED QUALIFICATIONS
Current Medical Assistant certification or registration preferred
High school graduate or GED equivalent preferred
Basic computer literacy
Knowledge of medical terminology
Good communication and customer service skills
Able to perform all Medical Office Assistant job functions and responsibilities
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $17.85 - $19.50
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
