This job will have the following responsibilities:

• Conduct assessments, create personalized treatment plans, and administer rehabilitative care to patients in their homes.


• Collaborate with healthcare professionals for comprehensive patient care.


• Document patient progress and communicate effectively with patients and their families.


• Stay updated on current physical therapy techniques and practices.
  
  

Qualifications & Requirements:

• Valid state license in physical therapy.


• Bachelor's or Master's degree in Physical Therapy.


• Previous home health or related experience preferred.


• Strong clinical, communication, and documentation skills.
