About the Role



We are seeking an Associate Patient Care Coordinator to support the Practice Support Unit (PSU). This role involves patient outreach, scheduling, and coordination of diagnostic follow-ups. The ideal candidate should be detail-oriented, proficient in customer service, and comfortable working in a fast-paced environment.


Key Responsibilities

• Conduct telephonic outreach to schedule patients for PSU health events
  
  


• Mail out and coordinate pick-up of A1c and FOBT kits
  
  


• Follow up on appointment rescheduling, cancellations, and no-shows
  
  


• Support the bus coordinator with market reports, attestation processing, and quality auditing
  
  


• Maintain accurate patient records and dispositions in WellMed system
  
  


• Ensure patients are not contacted more than six times within a 60-day period
  
  


• Handle inbound calls, addressing concerns and directing inquiries appropriately
  
  


• Meet daily productivity goals while maintaining professional relationships
  
  

Required Qualifications

• 
  High School Diploma or GED
  
  
  


• Strong communication and customer service skills
  
  


• Working knowledge of general office procedures
  
  


• Basic proficiency in Microsoft applications
  
  

Preferred Qualifications

• 
  1-2 years of experience in data entry or healthcare support
  
  
  


• ICD-10 and CPT coding experience
  
  


• Prior experience with Electronic Medical Records (EMR) systems
  
  


• Knowledge of medical terminology
