Hospice Rn Case Manager at The Judge Group Inc. in Oak Creek, Wisconsin

Job Description:

Location: Oak Creek, WI

Description: Are you an RN looking for a Case Manager role? If so, then keep reading!


Details:

• Case load is 12 patients in a 60-mile radius, seeing on average 4-6 per day




• RNs need live in their territory (in or near Oak Creek, WI, )
  
  


• Monday to Friday 8 AM to 5 PM with weekend and holiday rotations
  
  




• When working a Saturday and Sunday they will have the Thursday off prior to the weekend or Tuesday off after the weekend
  
  


• Have a large team and will be backup to the on-call team

Compensation:

• Base pay is commensurate and based on experience
  
  


• Bonuses for the following:
  
  


• Each Additional Visits
  
  




• Each Death Pronunciation
  
  


• Each Admission
  
  




• Reimbursements:
  
  




• Mileage (for legal purposes, the initial daily commute to the first place of visit is not reimbursed)
  
  




• Tablet or phone (if applicable)
  
  




• Scrubs are provided

Qualifications:

• A passion for hospice care and the ability to build relationships with both patients and their family members
  
  


• Current WI RN licensure and CPR certification
  
  


• The ability to work efficiently with little direct supervision
  
  


• A valid driver's license and an insured automobile that is in good working order
  
  


• Acute care experience in an institutional setting or home care experience is preferred, but not required. The client is excited to train new RN's that are passionate about hospice care!
