Summary:



The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility's Social Services Department. As a Social Worker, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.



Key Responsibilities:

• 
  
• Assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs.


• Participate in discharge planning, resident assessments, and developing social care plans.


• Interview residents/families to obtain social history.


• Refer residents/families to appropriate community agencies when needed.


• Provide consultation to staff and community agencies on meeting residents' social/emotional needs. Interpret residents' social, psychological, and emotional needs to the care team.


• Assist in obtaining resources from community agencies to meet residents' needs.


• Ensure confidentiality of residents' protected health information.

Education:



The job requires a Bachelor's degree in Social Work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure
