Engage members face-to-face or over the phone to discuss their need for provider follow-up.
Create a positive and impactful experience that drives Quality performance among assigned members.
Conduct outreach to promote participation in health-related programs, services, and provider visits.
Visit members' homes to connect with hard-to-reach populations and encourage provider appointments.
Collaborate with internal care teams, community partners, and healthcare providers to ensure member success.
Apply methods that encourage learning and foster positive behavioral changes in members.
Manage multiple tasks effectively, demonstrate self-motivation, and deliver quality results while meeting deadlines.
Continuously learn and respect the cultures and values within the community.
Preferred Qualifications:
Bachelor's degree in a behavioral health-related field.
Licensure or certification as an LPN, CNA/Home Health Aide, or Medical Assistant.
Experience working with patients with behavioral health needs.
Completion of a Community Health Worker (CHW) State course.
Background in managing populations with complex medical and behavioral needs.
Familiarity with arranging community resources and using electronic charting.
Knowledge of Medicaid and Medicare populations.
