Key Responsibilities:

• Engage members face-to-face or over the phone to discuss their need for provider follow-up.
• Create a positive and impactful experience that drives Quality performance among assigned members.
• Conduct outreach to promote participation in health-related programs, services, and provider visits.
• Visit members' homes to connect with hard-to-reach populations and encourage provider appointments.
• Collaborate with internal care teams, community partners, and healthcare providers to ensure member success.
• Apply methods that encourage learning and foster positive behavioral changes in members.
• Manage multiple tasks effectively, demonstrate self-motivation, and deliver quality results while meeting deadlines.
• Continuously learn and respect the cultures and values within the community.

Preferred Qualifications:

• Bachelor's degree in a behavioral health-related field.
• Licensure or certification as an LPN, CNA/Home Health Aide, or Medical Assistant.
• Experience working with patients with behavioral health needs.
• Completion of a Community Health Worker (CHW) State course.
• Background in managing populations with complex medical and behavioral needs.
• Familiarity with arranging community resources and using electronic charting.
• Knowledge of Medicaid and Medicare populations.
