About the Role



We are seeking a licensed Mental Health Professional (Therapist) to join our Assertive Community Treatment (ACT) team. This role involves assessing, planning, and coordinating treatment for clients and their families. Candidates must have a strong commitment to mental health care and will work closely with physicians, healthcare providers, and community resources to ensure comprehensive support for patients.










Licensure Requirement:

• Must possess one of the following licenses: LPC, LCPC, LMSW, LCSW, or LCSW-C.
  
  
  


• Candidates willing to sit for a licensure exam will be considered. The exam must be scheduled within 60 days of employment.
  
  

Key Responsibilities

• Conduct screening interviews and assessments to evaluate patients' mental health needs.
  
  


• Develop individualized treatment plans addressing medical, psychiatric, and psychosocial challenges.
  
  


• Provide individual, group, and family counseling based on treatment plans.
  
  


• Identify service gaps, advocate for patients, and ensure access to community resources.
  
  


• Collaborate with physicians, nurses, and social workers to coordinate patient care.
  
  


• Maintain accurate and compliant case management documentation per regulatory standards.
  
  


• Assist patients with daily living skills, housing, budgeting, and transportation needs.
  
  


• Manage crisis intervention, preventing hospitalization when possible.
  
  


• Organize and schedule medical appointments and therapy sessions.
  
  


• Provide education and counseling to families and natural supports.
  
  


• Deliver illness management strategies and symptom education based on evidence-based practices.
  
  


• Maintain high fidelity to the ACT Model of Care to ensure effective service delivery.
  
  

Qualifications




Education & Experience:

• 
  Master's degree in Social Work, Psychology, or Professional Counseling (preferred).
  
  


• 
  3+ years of post-master's clinical experience in a mental health setting.
  
  

Skills & Requirements:

• Must be a licensed Mental Health Professional in Maryland.
  
  


• Strong organizational, problem-solving, and communication skills.
  
  


• Ability to work independently while collaborating with a multidisciplinary team.
  
  


• Valid driver's license and clean driving record.
  
  


• Willingness to be available for crisis interventions and participate in on-call rotations.
