Position Summary:


The MDS Coordinator is responsible for the day-to-day coordination and oversight of all aspects of the RAI and Care Planning process in accordance with Federal and State


regulations.



Daily duties:

• Scheduling of MDS assessments


• Completion of MDSs


• Completion of RAPs


• Complete/update Care plans


• Utilize the Daily Care Plan Update sheet and work collaboratively with the nursing team to ensure implementation.


• Attend daily meetings as assigned


• Conduct Resident Care Conferences per facility schedule


• Communicate with interdisciplinary team members


• Track admissions and discharges


• Attend morning meeting/clinical report as assigned


• Computer Application/Data Entry

Weekly duties:

• Transmit MDS's to State


• Attend weekly resident "At Risk" meetings


• Work collaboratively with nursing team to properly utilize and implement the MDS Alert Sheet.

Monthly duties:

• Complete monthly calendar for MDS completion and Resident Care Conference 2. Attend and participate in the Quality Assessment and Assurance meeting


• Attend and participate in the QI/QM investigation meeting.


• Attend all monthly staff meetings

Quarterly Duties:

• Attend facility communication meetings


• Attend Regional MDS Coordinator meetings


• Other duties as directed by the Director of Nursing/Administrator

Recommended Minimum Position Qualifications:




Education:

• Graduate of an approved LPN/RN program.


• RN preferred.

Professional Experience:

• MDS Coordinator experience preferred.

Certification/Licensure:

• Actively licensed as an LPN/RN in assigned state(s) of employment.
