Full Time

GENERAL RESPONSIBILITIES:     

• Provide Target Case Management and Wraparound Facilitation to persons assigned to caseload.


• Facilitate meetings with treatment/wraparound teams.


• Develop comprehensive treatment plans for persons in service.


• Coordinate, monitor and/or complete all waiver paperwork, releases, and assessments.


• Work hours are flexible to meet the needs of the person and families served.


• Monitoring of progress of treatment plans and interventions and making changes as necessary.


• Coordination of care with community partners.

MINIMUM QUALIFICATIONS: 

• Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience.


• Excellent interpersonal and teamwork skills


• Excellent organizational and time management skills


• Interest and investment in working with children who experience Severe and Emotional Disturbance (SED) and their families.


• Have completed or will complete any required training within six months of employment.


• Must have a current and valid Kansas driver’s license.
