ILLINOIS MEDICALLY COMPLEX/DD FACILITIES

Summary:

We are seeking a dedicated healthcare professional to provide high-quality resident care, ensure regulatory compliance, and support staff development. This role involves assessing residents, coordinating care plans, training staff, and maintaining accurate documentation.

Key Responsibilities:

Conducts daily resident assessments and ensures proper care delivery.Develops and modifies care plans to meet residents needs.Communicates effectively with residents, families, and healthcare teams.Maintains accurate documentation of care, incidents, and condition changes.Trains and supports staff in compliance with facility policies and regulations.Ensures workplace safety and participates in emergency preparedness.

Qualifications:

Strong communication, organization, and leadership skills.Ability to lift up to 50 lbs and perform physical tasks.Knowledge of regulatory standards and best practices in resident care.

SUPERVISORY RESPONSIBILITIES
Directly supervises nursing staff, including hiring, training, performance appraisals, and problem resolution.

EDUCATION and/or EXPERIENCE
Graduate of LPN or RN nursing program with at least one year of related experience preferred.Meets state and federal nursing requirements.
