Provide pharmaceutical services to patients and medical staff at Shepherd Center with a focus on patient safety. A resident is a graduate pharmacist who is working in conjunction with preceptors and staff pharmacists while gaining practical experience in various settings. The resident is an employee of Shepherd Center and is eligible for benefits and paid time off.



SIGNIFICANT RESPONSIBILITIES

• Responsible for the safe, accurate and rational distribution of pharmaceutical agents to patients of all ages, employees and designated areas of the hospital, as well as the supervision of technical personnel. This shall be carried out in accordance with all state/federal laws, Joint Commission standards, and hospital/department policies and procedures.

• Accurately fills medication orders.


• Accurately enters orders and clinical information into computers and verifies physician-entered orders.


• Accurately checks technician work. Knows how to check for inventory in Pyxis, add inventory to Pyxis, unload drug from Pyxis stations, and interpret Pyxis controlled-substances usage reports in the absence of the Director.


• Maintains adequate control of controlled substances in the Pyxis C-II Safe.


• Checks crash carts within four hours after technicians restock them.


• Accurately calculates and compounds parenteral products.


• Washes hands frequently. Uses aseptic technique without exception.


• Enforces adherence to USP 797 guidelines.


• Complies with all federal and state laws and hospital/department policies and procedures.


• Adheres to highest standards of pharmacy practice and professionalism.


• Maintains thirty (30) hours of continuing education per two years in accordance with Georgia law.


• Prioritizes technical work for technical staff if needed.


• Informs Director of performance or discipline problems of technical staff.


• Provides continual guidance to technical staff as it applied to performance of pharmacy services.


• Interacts appropriately with physicians, nurses, and other professionals in regard to appropriate drug therapy. Follows through on all inappropriate drug regimens.


• Responds to non-formulary medication orders. Communicates with physicians, recommending appropriate formulary alternatives, and documents interaction in i-Vent.


• Ensures that allergy information is obtained on all inpatients prior to dispensing medication.


• Contacts physician concerning clinical drug interactions. Interacts proactively with nursing staff to assess and rectify service issues.


• Complies with formulary guidelines and restrictions.


• Responds promptly to physicians and nurses on medication questions.


• Documents questions, responses, and interventions if appropriate in i-Vent.


• Demonstrates expedience and professionalism in responses.


• Participates in one-on-one orientation of new personnel as assigned.


• Provide inservices for hospital staff and community education at the request of the Director.


• Prioritizes and complete pharmacy clinical services assigned by preceptors.


• Retrieves data for performance improvement monitoring and makes recommendations to improve care, resolve identified problems, and improve existing pharmacy programs.


• Completes all outstanding clinical activities by end of shift or communicates issues to next shift.


• Documents intervention and outcome.

JOB SCOPE



The pharmacist resident supervises all aspects of medication management in all areas of the hospital involving distribution, IV admixture, clinical programs and overseeing activities of pharmacy technicians. This includes selection/procurement of medications, storage of medications, ordering/transcribing and preparing/dispensing of medications, proper administration of medications, monitoring medication effects, and supervision of high-risk medications.


Clinical services will include pharmacokinetics, parenteral nutrition, IV to PO conversion, renal and hepatic dosing, antibiotic stewardship, opioid stewardship, and medication education for hospital staff and patients. The primary focus of the resident will be the clinical aspects of the position, including special projects as assigned by the preceptors, RPD, and/or DOP. The pharmacist resident will also assist in the management and education of undergraduate pharmacy students doing rotations at Shepherd Center along with the assigned preceptors.



RESIDENT DUTIES



The residency program is reviewed and accredited by the American Society for Health-Systems Pharmacists. The program has specific requirements for participation by the hospital and the residents. Shepherd Center residents will meet all ASHP qualifications.


The resident will be a graduate pharmacist either licensed or eligible for licensure shortly after hiring. The resident will staff an average of 6 hours per week after licensure. The resident will be expected to complete medication use evaluations, P&T monographs, and newsletter articles at the request of the preceptors or resident director. The resident will assist in precepting undergraduate pharmacy students. The resident will present a project at the Southeastern Residency Conference. The resident will be BLS and ACLS certified. The resident will attend professional meetings during his/her residency. The resident will be encouraged to sit for the Board of Pharmaceutical Specialties Board Certification (BCBPS).



MINIMUM QUALIFICATIONS REQUIRED

• 
  Education: Pharm D; BCPS, MBA or MHA a plus


• 
  Experience: Undergraduate clinical rotations a prerequisite; work experience in community or hospital pharmacy a plus


• 
  Licensure, Certification, and/or Registration: Georgia registered pharmacist, consultant pharmacist registration optional


• 
  Other Minimum: Computer skills


• 
  Qualifications: Excellent communication skills and a positive collaborative attitude with hospital staff, other departments, administration, and medical staff.

Must obtain and maintain a pharmacist license without restriction in the State of Georgia within 90 days of employment.
