Job Description:

A Registered Nurse (RN) provides care to residents in a skilled nursing center, under the direction of the Director of Nursing Services (DNS) and as prescribed by the physician, by assessing resident needs, administering medications and treatments, providing direction to care staff, partnering with members of the care team to achieve positive resident care outcomes, and in compliance with clinical policies and procedures, nursing practices, and local, state, and federal regulations.

Wage Range: $45-$55

ESSENTIAL DUTIES AND RESPONSIBILITIES

Performs assessments of residents, documents, and notifies Resident Care Manager (RCM), DNS, and/or physician of changes of condition and reaction to medications, treatments, and significant incidents. Takes temperature, pulse, blood pressure, and other vital signs, performs assessment of skin, and observes cognitive functioning and behaviors to detect deviations from baseline and assess the condition of residents. Performs nursing treatments and administers prescribed and PRN medication(s) in accordance with the resident care plan and as prescribed by the physician and with approved nursing techniques. Evaluates resident response to nursing interventions and informs RCM of observations. Documents all care provided and observations in a timely and accurate manner including charting assessments and vital signs, medications and treatments given, writing progress notes, and completing incident reports. Transcribes and enters orders into the resident's records and seeks clarification of unclear orders from the ordering physician and/or other health care professionals. Provides indirect supervision to care staff and licensed practical nurses (LPN) in the daily delivery of resident care, by managing workflow and assignments, providing feedback, problem solving concerns as they arise, and communicating with DNS regarding team member performance. Treats all residents, visitors, and staff with respect, dignity, and promotes resident rights. Displays exceptional customer service in all interactions. Assists with new admissions, transfers, and discharges. Provides comprehensive education to residents and residents' families. Promotes a positive physical and psychosocial environment for the resident that upholds residents' rights and protects residents from all forms of abuse. Reports all incidents of resident abuse or suspected abuse immediately and completes required incident reports and investigations. If suspicion of a crime is suspected, reports to survey agency and at least one local law enforcement entity within a designated time frame by e-mail, fax, or telephone. Participates as part of the care team in the development and implementation of the plan of care.

Communicates with the physician, members of the resident's care team, the resident, and family members to keep them informed of the resident's condition and the care provided. Notifies DNS, physician, and/or other healthcare professionals immediately of significant changes of condition. Follows all clinical policies and procedures, ensures infection control and safety practices are followed, maintains HIPAA compliance, and works within the scope of their license and practice. Attends and participates in continuing education programs keeping CEUs up to date and attends and participates in training, in-services, and staff meetings. Responds to resident call lights, and assists with transfers, ADLs, and other personal care tasks as needed. Maintains up-to-date knowledge of all rules, regulations, and policies pertaining to resident care. Responsible to stay informed of nursing practices and standards and apply new knowledge to meet the changing needs of residents. Performs other duties as assigned.

Maintains a safe and secure working environment and practices safe working habits.

QUALIFICATIONS AND EXPERIENCE

Must have a current, unencumbered Registered Nurse license in state of practice, or the ability to obtain one before practicing in the state. Experience working in skilled nursing including medication administration preferred. Ability to creatively problem solve resident and staff member situations. Must be a team player, well organized, and flexible.

EDUCATION

Associate's degree in nursing, Bachelor of Science in Nursing, or Master of Science in Nursing.

SUPERVISORY RESPONSIBILITIES

This position has direct supervisory responsibilities.

LANGUAGE SKILLS

Ability to communicate effectively, in English, with residents, visitors, and staff. Good written and oral communication skills. Ability to read and interpret documents, including safety rules, operating and maintenance instructions, and procedure manuals.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to maintain, understand, and review all reports and records.

REASONING ABILITY

Ability to accurately evaluate residents' needs and problem-solve to meet those needs. Ability to accurately assess, troubleshoot, and problem-solve needs and evaluate results.

WORK ENVIRONMENT

Works in a skilled nursing center in resident rooms, at the nursing station, and throughout the building. Regular in-person attendance is an essential function of this position.
