JOB SUMMARY

As the key Human Services Organization (HSO), VOACC will provide housing navigation support and sustaining services in Bladen, Pender, and Columbus counties in NC. The Program Director is responsible for overseeing the success of the program including its financial sustainability and ensuring that the case manager and peer support specialist are delivering the following services: inspection for housing safety and quality, housing move-in support, essential utility setup, one-time payment for security deposit and first month’s rent, as well as short-term post-hospitalization housing. The Program Director may also be responsible for carrying a caseload at times and delivering these services.

RESPONSIBILITIES:


  • Supervise overall Healthy Opportunities Program/staff per the HSO Manual

  • Participate in readiness reviews for program start-up

  • Use of NCCare360 for Pilot Service Referrals, Invoicing, and Reporting

  • Report service delivery and related outcomes

  • Invoice for delivered authorized pilot services through NCCares 360 (training will be provided)
    • Note: this must be accurate with supporting documentation and within the required timeframes per the Network Lead’s HSO Manual, Department Protocols, as well as maintained by all privacy and security requirements.

  • Support identification and enrollment of pilot-eligible individuals

  • Participate in pilot-related training and technical assistance efforts

  • Participate in pilot convenings

  • Participate in pilot quality improvement, program evaluation, and program integrity

  • Participate in pilot-related reporting and evaluation

  • Weekly documentation oversight to ensure accuracy and compliance with regulatory standards.

  • Transcribe documentation oversight into weekly invoicing oversight, ensuring timely and accurate invoicing processes.

  • Communicate invoicing oversight results to the Director of Operations and the finance team to ensure financial transparency and accountability.

  • Community Outreach engagement and strategic collaboration with other agencies, providers, and partners throughout our service area. 

  • Other duties as assigned

EFFECT ON END RESULT:


  • Program participants are provided with timely and effective assistance in the evaluation of their immediate and long-term housing needs.

  • Accurate and timely data collection provides current status of program participant progress.

  • Pilot Program provides needed feedback to contribute to the evaluation and efficacy of the pilot model design and its implementation.

REQUIREMENTS


  • A Bachelor’s degree in Human Services or related field; a Master’s degree is preferred

  • Experience required in communication, peer support, care coordination, case management, motivational interviewing, advocacy, and having social service connections where applicable.

  • Familiarity with the Housing First model, housing/homeless is desired.

  • CADC-R, CADC-A, or LCAS-A are preferred; VOACC will provide free clinical supervision as governed by the NCSAPPB board.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.

  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.

  • Operating office equipment requiring continuous or repetitive hand/arm movements.

  • The ability to remain in a sitting position for extended periods of time
