Job Summary
Summary


Responsible for improving the quality of patient interactions and overall satisfaction within the hospital. This role involves guiding patients and their families through their healthcare journey, addressing concerns, and ensuring that their needs are met in a compassionate and efficient manner.


Essential Functions


-Works closely with patients and families and a multi-disciplinary care team.


-Conducts needs assessments with patients to identify gaps and refers patients as appropriate.


-Organizes and leads team meetings, delivering updates on program performance, progress and strategic initiatives.


-Maintains accurate and up-to-date patient care records and program metrics.


-Analyzes data to identify trends and opportunities for program improvement.


-Collaborate on initiatives to enhance holistic care delivery, incorporating feedback from patients, families, and care team members.


-Foster and grow partnerships with external organizations and stakeholders to expand the program's reach and impact.



Qualifications



Education

Bachelor's Degree Related Field of Study required


Can this role accept experience in lieu of a degree?

Experience

Experience in a hospital setting, healthcare, or a related field 0-1 year required


Experience working with patients and families 0-1 year required


Experience in research and/or grant writing 1-2 years preferred



Knowledge, Skills and Abilities

- Strong organizational and project management skills.
- Excellent interpersonal and communication abilities to collaborate with diverse teams and build relationships with patients and families.
- Proficiency in data management, reporting and analysis.
- Proficiency with EPIC, Red Cap, and Microsoft Excel is desirable.
