Job Description:

Traditions Home Health Services is currently seeking compassionate, skilled, reliable, and experienced Home Health Aide for the Onset and Rochester area to assist with our Veteran male client.

Shifts Available:
3 days per week: 5-hour days, preferably 9a-2p Monday, Wednesday, and Friday.

Job Description:
- Assist with the Activities of Living, personal care
- Assist with Household support: light housekeeping, laundry, dishes
- Encourage walking, exercising
- Providing stimulating conversation
- Activities
- Encourage fluid intake, possible meal prep in accordance with orders/CarePlan
- Competently documenting shift activities frequently during the shift digitally using a smartphone
- Home Health Aide would report any concerns to the Traditions Home Health Services Office Support Team in a timely manner
