Location: Butler, PA

Salary: $60,000.00 USD Annually - $70,000.00 USD Annually

Description:
Our client is seeking a Home Health RN for Butler County, PA to join their team. If interested and qualified, please apply immediately!


Position Type: Field-based, work from home with patient visits


Schedule: Monday to Friday, typically day hours


Responsibilities:

• Deliver comprehensive skilled nursing care and manage patient cases.


• Identify needs for additional patient care services, make appropriate referrals, and coordinate overall patient care.


• Communicate admission information and patient condition changes to the Patient Care Coordinator (PCC), and implement agreed-upon plan of care adjustments.


• Provide timely and relevant patient/family education and support based on diagnosis and anticipated outcomes.


• Participate in quality assessment and improvement initiatives.


• Offer consistent, professional guidance and support to LPNs to foster their clinical growth and excellence.


• Maintain a positive attitude during periods of change, and support all organizational activities and processes.

Qualifications:

• Active PA RN License


• Valid PA Driver's license and reliable car


• Minimum 6 months of experience as an RN (any setting)


• Computer proficiency
