Work Location: This is an in-person role at The Oaks (630 S. Llewellyn Ave, Dallas, TX 75208) office.


Pay: $16,502 per year stipend (required # of hours: 1,700/year)

Medical, dental and vision paid for employee at 100%.

Day-To-Day

• Develops trusting relationships with clients by providing support and advocacy to help achieve health goals
• Conveys the purpose and services of Community Health to the member population and the impact that program or service.
• Meets member in clinic, facility, office, or at home to help identify social determinants of health impacting member's health and general well-being
• Follow-up with health management/care plans with both members and providers
• Coach member in effective management of their chronic health conditions and self-care.

Qualifications

• High School diploma or equivalent
  
• Any combination of 1 years health/social services experience and/or education
  
• Prefer candidates who live within a 10 mile radius of zip codes 75208 & 75211
• Valid Texas Driver's License and ability to be insured.
