Team Description: The candidate will be part of a Pediatric OBGYN team, which includes one other social worker and six RN case managers.
Required License/Certification: Social Worker License.
Purpose of Job: Responsible for coordinating social services for health plan members. Collaborates with healthcare delivery stakeholders and members throughout the continuum of care. Develops individualized plans that optimize the psychosocial and financial factors impacting care. Acts as a resource for the member and the healthcare delivery team, identifying and managing referrals to community resources.
Major Responsibilities:
Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
Manage social service plan of care throughout the continuum of care as a single point of contact, including outpatient services.
Document all member assessments and services provided, including referrals to social service programs and follow-up documentation to ensure member completion of referral.
Meet with members in their homes or at their providers' offices if requested, deemed appropriate, or warranted.
Perform other duties as assigned (e.g., assist health plan members in inpatient settings, assist the inpatient case management team(s) with resources to facilitate safe transitions of care, participate in Interdisciplinary Team meetings and Utilization Management rounds as necessary, etc.).
Qualifications:
Bachelor's Degree in Social Work required.
State of Nevada Social Work license required.
Minimum of two years of related social work experience in an acute care setting, clinic, home health, or case management setting.
Must demonstrate a working knowledge of social work theory and practice and how to use a variety of diagnostic tools and techniques.
Must have a basic knowledge of both state and federal programs and the community-based service continuum and the rules and regulations applicable to these programs and services.
Knowledge of advanced directives and legal regulations, including reporting of abuse, required.
Understanding of managed care preferred.

Skills in building and maintaining a community resource database by establishing effective liaisons with key sources of assistance for members and families.
Intermediate computer skills, including experience with Microsoft Word and the ability to navigate in a Windows environment.
Ability to communicate appropriately with diverse member populations and to be flexible and supportive of member and family needs.
Ability to work in varied settings with minimal supervision required.
