Department: Greater Rochester Independent Practice Association (GRIPA)
Location: 100 Kings Highway South, Rochester, NY
Hours Per Week: 40 hours
Schedule: Full-time, Days
This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH).
SUMMARY
The Social Work Care Manager will provide direct patient support both in-person and remote. They will be responsible for completing assessments, developing care plans, implementing interventions, and linking patients and families to community resources. The Social Work Care Manager will act as part of an interdisciplinary care team.
RESPONSIBILITIES
Work in settings across the care continuum including hospitals, provider practices and patient's home and/or community settings
Complete assessments that address social determinants of health
Develop a care plan and implement interventions
Provide psychosocial support
Provide timely documentation of interventions with, or on behalf of patients
Actively participate in on-line and in-person educational opportunities
Monitor and measure patient progress
REQUIRED QUALIFICATIONS
MSW or Masters in a related field or BSW with 5 (five) years of medical and/or community based Social Work/Care Management experience.
Knowledge of local community resources
Excellent communication and interpersonal skills
Ability to navigate complex medical and social systems
Exceptional documentation and planning skills
A collaborative and flexible spirit
Strong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)
PREFERRED QUALIFICATIONS
Home based service delivery experience
Bilingual skills
PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
PAY RANGE: $60,500.00 - $85,000.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
