Position Summary:

The Care Navigator works with the patient’s primary care team to address the complex non-medical needs that can negatively affect a person’s health and well being. The Care Navigator promotes patient-centered, comprehensive and coordinated care supporting the patient and their family in navigating medical, social service, and legal systems.

Essential Function​s​


  • Asses patient’s food security and connect to community food access resources such as local food pantries and provide information and assist with applying to SNAP (food stamps) as appropriate.

  • Connect parents with resources for subsidized childcare, playgroups, parenting classes, school enrollment, afterschool programs, and summer camps.

  • Provide 1:1 assistance to patients with form completion to access community resources and benefits when a literacy or language is a barrier.

  • Connect patients with legal and immigration resources, housing search information, financial assistance resources, education and job search resources.

  • Assist patients with navigating services of third-party agencies such as Inspectional Services, Massachusetts Commission for the Blind and Deaf/Hard of Hearing, MassHealth, Personal Care Attendant Services, Senior Services, Transportation Services and homeless shelters.

  • Collaborate with legal service agencies and educate patients on their rights as tenants, workers.

  • Connect patients to and assist with accessing Social Security Disability and Department of Transitional assistance benefits.

  • Assist patients with SSI application process including interview accompaniment, application completion and process follow up.

  • Connect patients to community based mental health services, including in-home therapy, outpatient therapy and psychiatry services, substance abuse services and domestic violence services.

  • Coordinate and support health promotions programs such as Clothing, food and Car Seat distributions.

  • Provide rental assistance support through the completion of appropriate applications.  Educate patient on housing search process and connect to resources.

  • Support maintain and up to date, comprehensive community resource database. Work to improve Providers knowledge of community resources and navigating SharePoint resources.es.

  • Participate in professional development and team trainings to further understanding of both community resources and the intersections of social justice and community health work.

  • Serve as liaison between NeighborHealth and community agencies.

  • Provide support to fellows and student interns

  • Maintain excellent punctuality and attendance.

  • Promote a sense of “team work” through demonstration of self-direction and self-motivation.  Solve problems independently or knows when to seek consultation.

  • Displays outstanding customer service skills when interacting with all NeighborHealth customers according to the ACE model.

  • Adheres to all NeighborHealth and departmental policies and procedures.

  • Participate in required departmental activities and meetings.

  • Perform other related duties.

  • Reads, understands and complies with all Standards of Conduct, JCAHO policies, CMS Pace Program Regulations and Standards, applicable State regulations and guidelines, ESP policies and procedures and all laws related to position responsibilities and employment at NeighborHealth;

  • Upholds procedures and systems to safeguard the confidentiality of   all patient and employee information.

  • Upholds safety policies, practices, and procedures including safety, fire safety, electrical safety, proper body mechanics and material     handling, office/ergonomic safety and other employee safety measures.

EDUCATION:  

  • Bachelor Degree preferred

EXPERIENCE:


  • 1-2 years in a direct service role. Experience with and underserved population a plus

  • Skills/Abilities: 

  • Strong second language skills. 

  • Spanish preferred.
