About the Role: We are looking for innovative health coaches with experience in senior care to join our program. Through partnerships with insurers, we provide support services to aging adults and their family caregivers. As a health coach, you will offer telephonic support, coordinate interventions such as education and assistive devices, and facilitate dynamic conversations with members. You should be proactive, follow program protocols, manage your time effectively, and be open to new approaches in senior support. Comfort with a startup mentality and adaptability to changing methods are essential.


Responsibilities:

• Collaborate in a team environment to educate, plan, implement, and evaluate interventions tailored to each member's needs.


• Handle day-to-day responsibilities:
  
  
  • Answer inbound and make outbound calls to members and their families, using motivational interviewing to support successful aging in place.
  
  
  • Evaluate members over the phone using a predefined tool to identify, assess, and manage interventions.
  
  
  • Coach members to identify needs and utilize interventions provided.
  
  
  • Document all interventions, contacts, and activities in our electronic records and billing system.
  
  
  • Build positive relationships with members and caregivers.
  
  
  
  


• Provide feedback to refine service offerings and engagement techniques.


• Participate in programmatic testing and offer feedback on new processes and materials.

Qualifications:

• Certification in health coaching and at least 2 years of experience using motivational interviewing.


• 2-5 years of experience working with older adults.


• Knowledge of the elder continuum of care.


• Proficiency with software and technology tools that support aging in place (Mac experience required).


• Experience in a fast-paced, dynamic startup environment is preferred.


• Ability to work 9:00 AM - 5:30 PM PST.


• Familiarity with long-term care insurance is preferred.

• Excellent interpersonal, listening, written, and verbal communication skills.


• Ability to establish effective working relationships with members and the team.


• Ability to work independently and within a team management system.


• Excellent documentation skills.


• Ability to learn and adapt to new technology.
