Description:

Job Title: HouseCalls Nurse Practitioner (NP)



Location: Denver County, CO



Hours: Create your own schedule, Monday-Sunday, 8:00 AM - 8:00 PM (Flexible weekend hours available)



Responsibilities:

• Conduct in-home health assessments for Medicare Advantage members, including:




• Reviewing past medical history, systems, and medications.


• Performing physical exams (e.g., monofilament tests, urine dipsticks, foot exams).


• Educating members on disease processes and medication compliance.




• Document findings electronically during and after visits (completed within 48 hours).


• Communicate results to PCPs and identify opportunities for clinical referrals.


• Ensure compliance with HIPAA regulations.

Qualifications:

• Licenses & Certifications:




• Active NP license (unencumbered).


• National Board Certification through AANP or ANCC (Family, Adult, or Geriatric specialties only).


• BLS/CPR certification.




• 
  Experience:
  




• Minimum 6 months as a Nurse Practitioner (no new graduates).


• Preferred: experience in geriatrics, home healthcare, or related fields.




• 
  Additional Requirements:
  




• Reliable transportation.


• Proficient with electronic medical records and internet use.
