Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.



Pay: $72,500 - $76,000 annually



Schedule: Fulltime, M-F, 8a-5p

The Home Care Coordinator will oversee the day-to-day operations and operational effectiveness of the Home Care department for Senior CommUnity Care. Participates in the Interdisciplinary Team (IDT) Meeting as required and acts as the Home Care Coordinator representative. Provides direct supervision to all home care Personal Care Attendants and C.N.A.s.



QUALIFICATIONS:



  • Current Licensed Practical Nurse in the state of Colorado require.

  • Cardiopulmonary Resuscitation (CPR) certification required.

  • A minimum of one year's experience in working with the frail or elderly population required.

  • Minimum of two (2) year's nurse management experience in a hospital, nursing home or community-based setting. Home health experience preferred.

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.

  • Must have a valid driver's license, proof of insurance and have reliable means of transportation.


Skills/Knowledge:



  • Broad knowledge base of health care problems of the frail elderly.

  • Knowledge of medical equipment and instruments.

  • Knowledge of common safety hazards and precautions to establish a safe working environment.

  • Skilled in identifying problems and recommending solutions.

  • Skilled in preparing and maintaining records and writing reports and responding to correspondence.

  • Skilled in developing and maintaining department quality assurance.

  • Skilled in establishing and maintaining effective working relationships with participants, staff members and family caregivers.

  • Ability to react calmly and effectively in emergency situations.

  • Knowledge of home care services nursing principles and practices, with particular reference to the elderly.


ESSENTIAL FUNCTIONS:



  • Directly responsible for the oversight and supervision of all home care aides and home care scheduler/ lead C.N.A. including schedule management, and overall operational effectiveness of the department.

  • Performs orientation, initial and annual competencies, in responsible for evaluating performance of all home care staff and schedulers.

  • Performs quarterly supervisory visits to home care aides in participants homes.

  • Educates home care aides on best practices in Home and Day Center caregiving practices.

  • Prioritizes utilization of homecare staffing needs to reflect actual enrollee homecare needs.

  • Performs initial, semi-annual & change of status assessments of participants in their home environment to determine their individual needs and coordinates plan of care with care-giving resources. Coordinates with the Interdisciplinary Team to develop plan of care for each participant. Ensures homecare staff follow the participant's plan of care as approved by IDT.

  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings, communicates participant changes, collaborates on plan of care decisions and coordination of twenty-four (24) hour care delivery.

  • Maintains participants' medical record and fulfills Senior CommUnity Care charting and reporting requirements.

  • Monitors participant medication adherence, administers medications and provides training in self-administration of all non-scheduled medications as needed.

  • Provides health counseling to participants and their families on an individual or group basis as needed.

  • Provides support to the program by working on-call in rotation as assigned, including weekend on-call.

  • Establishes and maintains a productive working relationship with relevant contracted providers and agency staff.

  • Engages in team building. Coaches and mentor's staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Assists in the establishment and enforcement of organizational policies and procedures.

  • Adheres to and is a role model of behavior consistent with VOA Code of Conduct and Senior CommUnity Care Standards of Service Excellence. Demonstrates ability to provide direct feedback to staff about performance, attitude and adherence to VOA Code of Conduct and Senior CommUnity Care Standards of Service Excellence.\

  • Acts within the scope of his or her authority to practice.

  • Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.

  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.

  • Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.

  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

  • Performs other duties as required or requested.
