Care Management Support Coordinator



BCforward is currently seeking a highly motivated Care Management Support Coordinator for an opportunity in Remote (Delaware)!

Pay Range: [$20] - [$22]

Job Description:




Position Purpose:



The Care Management Support Coordinator plays a vital role in supporting administrative care management activities. This position involves direct outreach to members, answering inbound calls, scheduling services, and connecting members to healthcare resources. The coordinator serves as a key point of contact for members, providers, and internal teams, ensuring seamless communication and documentation in compliance with state and regulatory guidelines.



Key Responsibilities:





  • Conduct outreach to members via phone to assist with care plan follow-ups, scheduling, and education.

  • Provide guidance to members on available health plan and community resources.

  • Serve as frontline support for member and provider inquiries, explaining care plans, procedures, and protocols.

  • Assist with member onboarding by sending welcome letters, program materials, and other related correspondence.

  • Maintain and update non-clinical member records to align with regulatory standards.

  • Identify members without a Primary Care Provider (PCP) and assist with obtaining one.

  • Screen members for eligibility in Care Coordination services and enter assessments into the system.

  • Address Social Determinants of Health (SDOH) needs by referring members to relevant services.

  • Adhere to company policies, procedures, and compliance standards.

  • Perform other duties as assigned.




Qualifications & Requirements:




Education & Experience:






  • Required: High School diploma or GED


  • Preferred: 2-3 years of experience in a managed care setting, medical office, or healthcare facility.




Skills & Competencies:





  • Experience handling high call volumes (50+ inbound/outbound calls per day).

  • Strong communication skills to interact effectively with members and providers.

  • Proficiency in computer systems and data entry.

  • Knowledge of medical terminology (preferred).
