The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high quality, low cost, innovative community-based ambulatory care. Are you a compassionate and skilled Social Worker with an interest in supporting pediatric patients and their families? We are seeking a dedicated Social Worker to join our team, providing comprehensive support to children and their families across Revere, Charlestown, and Chelsea, MA. This role is four days on-site and one remote day per week, ensuring meaningful, hands-on engagement with the communities we serve.

Job Summary
Conduct psychosocial assessments to identify emotional, social, and environmental factors affecting pediatric patients and their families.
Develop individualized treatment and care plans in collaboration with healthcare teams and pediatrician.
Advocate for families by connecting them with community resources, financial assistance programs, and specialized services.
Monitor, evaluate, and document patient progress and treatment outcomes.
Collaborate with healthcare providers, educators, and community organizations to ensure seamless patient care.
Qualifications
Education: Master's Degree in Social Work or Mental Health & Behavioral Medicine (required).
Licensure: MA LCSW, LICSW, or LMHC.
Experience: At least 1 year of related clinical experience in a medical or pediatric setting.
Additional Job Details (if applicable)
Knowledge of pediatric community resources and social services strongly preferred. Bilingual in Spanish preferred but not necessary.
