Position Title: Program Coordinator, Faith Community Network
Position Overview:
The Program Coordinator for the Faith Community Network Program at Holy Cross Health serves as the primary liaison between the health system and diverse faith-based communities, facilitating health education, connecting communities to essential resources, and advancing well-being by addressing the social determinants of health. Recognizing the strong connection between spirituality and health, this role partners with faith leaders and congregations to integrate holistic wellness into their ministries, empowering communities to address maternal health, diabetes prevention, chronic disease management, and mental health awareness. Supports the mission and core values of Trinity Health and Holy Cross Health.
Position Functions:
In addition to leading the Faith Community Network Program, the Program Coordinator will also support other community health initiatives at Holy Cross Health, contributing to efforts related to social care, food security, and chronic disease prevention.
Key Responsibilities
Faith Community Network Coordination
Establish and maintain relationships with faith-based communities across all denominations and backgrounds, fostering partnerships that recognize the vital role of spirituality in health and healing.
Organize and facilitate Mental Health First Aid training and coordinate the certification of Health Ministers to strengthen faith-based health ministries.
Connect faith communities to Holy Cross Health (HCH) resources, including programs on diabetes prevention, chronic disease management, and mental health awareness.
Provide technical assistance, training, and resources to congregations interested in developing or expanding health ministries.
Program Implementation & Support
Collaborate with community partners and internal stakeholders to align faith-based programs with broader community health initiatives.
Plan and coordinate health-focused events, educational workshops, and resource distribution in partnership with faith communities, ensuring spiritual and cultural relevance.
Track program participation, collect impact data, and prepare reports for internal and external stakeholders.
Participate in strategic planning, grant reporting, and partnership development to enhance community engagement efforts.
Support cross-functional initiatives to improve whole-person health, recognizing the mind-body-spirit connection.
Bachelors degree from an accredited college/university in community health or a related field; master's degree in public health or related field preferred.
Minimum Knowledge, Skills, and Abilities Required:
Two years' experience in a related position. Excellent organizational skills with proven ability to manage multiple projects. Excellent interpersonal skills, with the ability to develop effective relationships with all levels of management as well as contract staff and a range of external constituents. Strong organizational and project management skills with the ability to work independently and recognize opportunities for program development Knowledge and experience in evaluation, data collection, analysis, and reporting with strong written and verbal communications skills. Proficiency in Microsoft Office software. Bilingual in English/Spanish preferred.
Pay Range:$25.17-$35.23
Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
