Description:
The Judge Group is looking for Nurse Practitioners to conduct in-home health assessments in Merrimack, NH with one of our largest clients nationwide.

• Per diem position offers you the chance to work independently, with compensation for each assessment, and flexibility with scheduling.


• Pay is dependent upon experience as a provider.


• Mileage is reimbursed at the federal rate

Overview:

• Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.

• Provide education on medication management, disease processes, and preventive care.

• Identify and document diagnoses to support care management and active treatment.

• Collaborate with primary care providers to communicate findings and address potential gaps in patient care.

• Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.

Minimum Requirements:

• Completion of at least 20 hours of in-home assessments per month.

• Valid Nurse Practitioner license in the state of New Hampshire.

• Board Certification from AANP or ANCC in Family, Adult-Gerontology, or a related specialty.

• Minimum of 1 year of clinical experience; experience with geriatric populations preferred.

• Strong communication skills and a commitment to delivering compassionate care.

• Reliable transportation for travel within the assigned areas.
